Just a minor update that improves how the “Next Year Placement” grade is handled. It now features a pull-down menu that will last up to 2025.
Grades K to 3 v1.86 Fillable
Grades 4 to 9 v1.87 Fillable
Here is a new version of the grades 4 to 9 form with a slight adjustment to the Applied Skills area. The adjustment only affects Don Ross users, all others can stick with 1.84 if they like.
Grades 4 to 9 v1.85 Fillable
The CSL committee approved a few minor changes to accommodate the new student self assessment. In the competencies areas include some “starter” language which you can edit and adapt for your own purposes.
If you have already started your June report cards with the existing template, just be sure to adapt it somehow to include student self assessment.

Please let me know if you have any problems with these forms.
- Fixed a script bug that prevented the ADST letter grade boxes from showing in the 4-9 form.
- Added a “2” button that allows the text field with rating scale to be shown/hidden for all subjects.
